At our clinic, we provide epithelium-off treatment, a method involving the creation of a corneal flap in the delicate outer layer (epithelium) of the cornea. This technique facilitates the deeper penetration of the liquid Riboflavin (Vitamin B12) into the corneal tissue during exposure to UV light. This non-invasive approach not only eliminates the necessity for a corneal graft but also has the potential to address some of the patient’s prescription needs.

The process of photopolymerisation, induced by the addition of Riboflavin under UV light, is pivotal in arresting the progression of the condition. This mechanism generates oxygen radicals, enabling the formation of robust collagen bonds. By strengthening these chemical bonds in the cornea, the procedure effectively augments thickness and stabilises the cornea, thus impeding the disease’s advancement. Notably, this treatment typically requires only a single session to effectuate a lasting change.

To be considered eligible for Collagen Cross Linking, individuals must have progressive Keratoconus that hasn’t reached an advanced stage necessitating a corneal graft. Additionally, candidates must be at least 18 years old. Determination of suitability for treatment will be made by the surgeon following a thorough assessment during the consultation.

Treatment Process

Each eye is treated individually, with a one week gap between procedures. The entire procedure is non-invasive and typically lasts about 60 minutes per eye.

Firstly, anaesthetic drops are applied to numb the eye, ensuring patient comfort throughout the treatment. An eyelid guard is then employed to prevent blinking during the procedure. Next, a weak alcohol solution is delicately applied to soften the upper epithelial cells, which are subsequently manually removed by our skilled surgeon.

Following this, a Riboflavin (Vitamin B2) solution is carefully dropped onto the surface of the eye, facilitating the cross-linking process. The eye is then exposed to a controlled amount of UV light. This process of adding drops and exposing the eye to UV light is repeated as necessary.

After the procedure, a protective contact lens is placed in the eye to safeguard it until the first post-operative check. Our specialist nurse will provide detailed instructions regarding precautions and medications essential for aiding your recovery. Your first post-operative check is scheduled four days after treatment to monitor progress and ensure optimal healing.

Aftercare

Upon receiving Collagen Cross Linking treatment at our UK clinic, you’ll be provided with an eye cover for the treated eye, along with anti-inflammatory, antibiotic, and hydrating drops. Our staff will instruct you on the appropriate usage of these drops. At the first post-operative check, scheduled 4 to 5 days after treatment, our surgeon will carefully examine the treated eye and remove the protective lens.

During the recovery period, which may last up to a week, patients may experience some discomfort for 2 to 3 days following the treatment. Dry eyes are common during the healing process and are considered a normal part of recovery. Visual fluctuations can persist for 1 to 3 months until the prescription stabilises, at which point new glasses or contact lenses can be fitted.

While most patients experience stability in vision after this period, minor fluctuations may continue for up to six months post-treatment. Regular post-operative checks are necessary throughout the year, starting from 3 months after treatment. These checks can be conducted either at our clinic or at one of our optometrist network partners.

Following treatment, patients can typically resume normal activities, although certain restrictions may apply, which will be discussed during the consultation. Driving is usually permitted once the prescription has stabilised and new glasses or contact lenses have been obtained. For patients using Kerasoft lenses, the waiting period may be reduced to 5 to 10 days before resuming driving activities.
